Participation Acknowledgment

I am the participant’s parent or legal guardian and give permission for the minor listed above to participate in the MakePlay Social activity. I understand activities may involve craft supplies, adhesives, beads, charms, gems, slime or sensory ingredients, liquids, small parts and other creative materials appropriate to the selected activity. I understand that hands-on activities involve ordinary risks, which may include spills, slips, minor cuts, skin irritation, allergic reactions, choking hazards from small parts or other accidental injury. I agree that the participant will follow MakePlay Social staff instructions and use all materials only as directed. I understand that MakePlay Social provides an activity experience and does not assume custody of participating minors; a responsible adult must remain available to supervise. To the extent permitted by law, I voluntarily assume the ordinary risks associated with participation and agree not to hold MakePlay Social LLC responsible for injuries or losses arising from those inherent risks, except where liability cannot legally be waived.
